Medicare Australia Chronic Disease Management Plans: What are they and am I eligible?
Articulate is proud to be an approved provider for Medicare-funded services under the Chronic Disease Management Plan (CDMP) program (formerly Enhanced Primary Care). Under this program, Medicare provides rebates for eligible patients to access Accredited Exercise Physiology services as part of a coordinated care plan developed by their GP.
A CDMP is designed for children and adults living with a chronic medical condition. It allows your GP to refer you to allied health professionals — including Exercise Physiologists — to support the management of long-term conditions through structured, evidence-based care.

Your doctor will work with you to develop a tailored CDMP based on your individual needs. If you have a chronic disease, Medicare may pay for some of the costs of your treatment through the CDMP.

How do we charge for CDMPs?
Our Medicare appointments are processed at our standard Exercise Physiology pricing.
The Medicare rebate is processed through our Tyro (Hicaps) terminal at time of payment straight back into your bank account (or with a receipt issued for you to claim through Medicare yourself). Unfortunately we are not able to offer bulk billed appointments.

Can I claim on my Private Health as well?
Unfortunately you are unable to claim both your Private Health and Medicare rebates on the same appointments. Some clients choose to use their private health to make the most of their longer initial appointments before moving on to shorter appointments on their plan after checking in with their plan. We are more than happy to work with whatever you might need.

- Strength, mobility and functional capacity improvement

Are you eligible for a chronic disease management plan?
Medicare offers CDMPs to people with eligible chronic diseases. To be eligible for a CDMP, you must have a chronic disease that is:
- Permanent or long-term
- Likely to last for six months or more
- Unable to be managed solely by your GP.
Your GP will assess whether you are eligible for a CDMP and can provide you with more information. Examples of conditions that are eligible for a CDMP include :
- Diabetes
- Heart disease
- Asthma
- Strokes
- Osteoporosis
- Arthritis

What does a CDMP entitle me to for exercise physiology?
If you are eligible for a CDMP, it may entitle you to:
- A set number of allied health services each calendar year (up to 5) including physiotherapy and exercise physiology
- A rebate of $61.80 per appointment if you are below the Medicare threshold for the year
- A rebate of 80% of out-of-pocket costs if you are past your Medicare Safety Threshold for the calendar year
