Distal Biceps Tendon Repair Rehabilitation
What is a distal biceps tendon rupture?
The biceps muscle has two attachments at the shoulder (the proximal tendons) and one attachment at the elbow — the distal biceps tendon, which inserts into the radial tuberosity on the radius bone. This distal attachment is critical for two functions: elbow flexion (bending the arm) and forearm supination (rotating the forearm so the palm faces upward) — the motion used for turning a screwdriver or opening a jar. When the distal biceps tendon ruptures, both of these functions are significantly weakened.
The injury occurs almost exclusively in men between 40 and 60 years of age, typically during a sudden eccentric load — the biceps contracts hard to prevent the elbow from straightening under an unexpected weight, and the tendon tears at its attachment to the radial tuberosity. Common scenarios include lifting a heavy box that is heavier than expected, catching a falling weight, or having a load suddenly shift during manual work.
The classic sign is the "Popeye deformity" — the biceps muscle retracts up the arm, creating a visible bunching in the upper arm and a loss of the normal contour in the front of the elbow. Not all ruptures produce a dramatic deformity, but most will show some asymmetry compared to the unaffected side.
Is surgery always required?
For complete distal biceps tendon ruptures — which constitute the majority of cases — surgical repair is generally recommended, particularly for active individuals and manual workers, because the functional deficit from a non-repaired complete rupture is significant and permanent. Supination strength can be reduced by up to 40% and flexion strength by up to 30% without repair. For older, less active individuals or those with significant medical comorbidities, conservative management is occasionally chosen, but this should be a considered decision made in consultation with an orthopaedic surgeon.
Partial tears — where the tendon is damaged but not completely detached — can sometimes be managed conservatively with rest and physiotherapy, though a proportion ultimately require surgical intervention if symptoms persist.
The timing of repair matters. Early repair — within two to three weeks of rupture — is technically easier and produces better outcomes than delayed repair, as the tendon retracts and the surrounding tissue becomes more fibrotic with time. What does surgery involve?
The most common surgical approach is a single-incision technique, where the tendon is reattached to the radial tuberosity through a small incision at the front of the elbow using suture anchors or a bone tunnel technique. A two-incision technique is used by some surgeons and may be preferred in certain anatomical situations. The choice of technique influences the approach to early rehabilitation — specifically the restrictions around forearm rotation in the early post-operative period.
What does rehabilitation involve?
The rehabilitation after distal biceps repair is a careful balance between protecting the healing tendon attachment and preventing the elbow stiffness that develops rapidly when the joint is immobilised.
In the first two weeks, the elbow is typically held in a position of flexion and supination in a splint or brace to protect the repair. Range-of-motion exercises within the surgeon's prescribed limits begin early — typically gentle active flexion and extension within a protected arc, and forearm rotation restrictions that depend on the surgical technique used. Shoulder, wrist and hand exercises maintain mobility in the surrounding joints.
From two to six weeks, the range-of-motion arc expands progressively and the brace is gradually weaned. This is a critical period for preventing stiffness — the elbow capsule tightens rapidly after trauma and surgery, and consistent daily range-of-motion work during this phase is the most important predictor of final range of motion achieved. Elbow extension is often the last range to fully recover and requires specific attention.
From six to twelve weeks, gentle strengthening begins once the tendon attachment has consolidated sufficiently to tolerate load. Biceps activation starts with isometric contractions, progressing to light resisted flexion and supination. This phase requires patience — the temptation to load the biceps before the repair is mature enough is one of the most common causes of re-rupture.
From three to six months, progressive strengthening toward functional demands continues. Return to manual work involving heavy lifting typically requires four to six months. Return to sport depends on the demands of the specific activity — contact sports and heavy strength training may take six months or more.
One important consideration specific to this injury is the forearm supination component of rehabilitation. Many patients focus on restoring elbow flexion strength and neglect supination, which is the function most uniquely dependent on the distal biceps. Targeted supination strengthening — using a pronated hammer or resistance band supination exercises — should be a consistent feature of the program from the mid-rehabilitation phase onward.
