Once we receive your referral we will contact your patient promptly to help them find a suitable appointment time. Patients are also welcome to book directly online or through our reception team at any time.

Referrals can be submitted via:
Online form — use the secure upload form below to submit patient details and any relevant documentation directly.
Email — [email protected]
Fax — 07 3036 6644
Phone — 07 3706 3407 — our reception team is available during clinic hours and happy to discuss referral details directly.
What to include
Patient name, date of birth and contact details (phone and/or email) are the minimum required to make contact and arrange an appointment. Any relevant clinical information — diagnosis, imaging reports, surgical notes, medication list, relevant history — is welcome and will be reviewed before the initial appointment. For CDMP referrals, please include the GP Management Plan and Team Care Arrangement documentation.
Referral types we accept
We accept referrals and funding through all major pathways including private health insurance, self-funded, Medicare Chronic Disease Management Plans (CDMPs) and Enhanced Primary Care (EPC), DVA, WorkCover Queensland, CTP insurance, and NDIS.
Who to refer to
Our team has specific areas of clinical expertise that are worth knowing about when referring. If you are unsure which clinician is most appropriate, selecting "Any" on the referral form or calling reception is the easiest approach — our team will match the patient to the most appropriate clinician based on their presentation.
